Fees

Dr. Farwell and Dr. Tedder are private pay practitioners only. In an effort to provide the highest quality in patient care, our practice does not accept insurance. Payment for services can be made by cash, check, Visa, Mastercard, American Express, or HSA/FSA cards. We keep a card on file regardless of the payment modality.
Out-of-Network Options
Dr. Farwell and Dr. Tedder are considered out of network providers.
We recommend potential clients contact their insurance provider for out-of-network benefits. After services are complete, we can provide a superbill for you to submit to your insurance.
Common CPT codes we use include 90791 for the intake appointment, 96132, 96133, 96136, 96137, 96138, and 96139 for testing, and 90837, 90846, and 90847 for psychotherapy. You may have a deductible to meet, or you may have a co-pay or co-insurance plan, and every plan is different.
We can provide you with a superbill, which you can file with your insurance carrier for out-of-network benefits. The way this typically works is the client pays a full fee out of pocket to Meet Your Mind, and once the full fee is received, we can provide you a superbill to submit to your insurance carrier, who will reimburse you directly. It is recommended that you contact your insurance carrier to verify out-of-network benefits and the process of submitting out-of-network provider claims for reimbursement.

**Frequently Asked Questions regarding focused evaluations:
§ All evaluations investigate potential diagnostic considerations based on your symptoms and reported concerns, even if your initial referral question is relatively straightforward.
§ Your symptom presentation may indicate broader executive function struggles, anxiety, autism, or another symptom constellation/differential diagnosis altogether, such as depression, etc., contributing to issues with attention and focus.
§ These considerations related to your initial presenting concerns will be discussed further at the initial intake appointment to clarify the scope and pricing when focused evaluations are requested.
